I was once seeing a baby for ER follow up, and I pulled up the child's X ray from the visit. The radiologist meant to call for "further evaluation" because the adult facility techs had not done a great job with the baby. 

Speech to text had transcribed "further violation needed." I read that to the mother and she nearly fell off her chair laughing.

"That's exactly what it was!" she said.

And that's why I never used speech to text either.
