If you mean what portion knows that the US Constitution outlines a republic rather than a direct democracy, I would guess most people.  They just use 'democracy' as short hand for 'representative democracy'.

If you mean what portion knows that we have a ruling elite that subverts the will of the people, probably a much smaller, but quickly growing portion.
