 I've let myself go. I am afraid of the pain of discipline.  
 At least you have identified a problem. I have slacked so much on self discipline myself over the past 4 years, and didn't even realize it until recently. My #1 priority the past few months is getting small habits to really stick. Some has been personal and plenty has been "professional" too. I'm a very baby steps person.  
 I'm doing that now, too. Step by step. It sucks. But it's better than not trying...  
 For sure. Do you track your metrics? I finally convinced myself to start tracking habits. It helps when I don't have anyone to share my progress with, history of accomplishment and so on. I don't like apps on my phone but I really needed something so I started using this  and setting reminders and such. I also still use a whiteboard if you want to avoid technology lol
