It means the server you are using didn't like the file and rejected it.

Have you tried a different media server?
nostr.build is the one I am using to circumnavigate this error. But I used to upload stuff on amethyst or other client before without nostr.build. The issue started after I removed affected relays due to spams. Not sure if it is related. 🤔
