The worst incidents always come from engineers who have the attitude of, "it's not a big deal if my system fails, because someone else's system will prevent an incident."  Chain enough people like that together on a project and you have an accident waiting to happen. 
 @8c23d0c5 I was always promoting self-criticism in every organization I worked at. Some of my Fukushima conclusions, for example, were directed towards our own practices. I thought inspectors spent too little time on site. How can you discover issues if you need a guide to find your way around the plant? I spent every outage sniffing around Olkiluoto with a camera, photographing loose connections etc. Went through every pipeline, memorized every valve, and so on. I wanted to KNOW the plants.
