 This may be an incredibly simplistic and possibly stupid question, but if drugs like Sudafed don't work (which I've known for years, and why I don't bother taking them), why did the FDA approve them in the first place?

The reality us that nearly all health and medical studies are done on a frustratingly small number of people. Results are easily skewed, and nearly always biased. A single case study of a handful of people can stand for years (decades) before further analysis is done… largely because the only funding for most studies is profit-driven.
