 A domain name where you can access the root folder and that folder is accessible to the wider web. 

 Urm. Not sure. I could probably explain it better when I'm not at work, but in your web servers .well-known folder you put a file called nostr.json. 

This is url for sovbit.host and you can see the format at least.
