At the base level, you make attestations about how much you trust particular people on particular topics, and your own subjective web of influence builds up from that (taking into account who _they_ trust, and how much, etc. many hops out).

It's not gameable or abusable because the reputation flows from you outward, at your control. 

I've been cheerleading this project pretty aggressively because I had worked on a similar paradigm on different platform. I got excited to bring the concept to Nostr and then what do you know, I find that David has already done it! Great!

 A proper subjective web of trust does not have that problem. There's no reason you need to assign any trust at all to "elites". The trust graph is yours and you decide who the elites are, from your point of view:
