Mag is full, rounds feed just fine, one round comes out another feeds to the top... but somehow, despite the fact that the follower is pressed tightly against the bottom round, and that pressure transfers all the way to the top round, some how there's a round loose and floating around in there (sound on to hear it)...

The audio isn’t coming through for me on multiple clients so I can’t really tell what’s going on from the video, but it could be that something got between the follower and the baseplate where the spring is. You could try emptying the magazine then taking it apart by removing the baseplate.
If you’ve never done that before there should be some sort of button on the bottom (you might need some sort of tool like a screwdriver that fits to push it in), push that in and slowly slide the baseplate forward. Be careful not to let the spring shoot everything across the room though 😂
Just got the audio on my end, my AirPods were doing something weird.
I’d guess it’s what I just mentioned. There’s also the chance the wrong caliber worked its way in somehow, but I wouldn’t expect that
Double checked and audio is there, but you just hear the round clicking around in there.
I did take the thing apart and checked everything the first time it happened. Nothing in there, no sign of what's going on.
It happens maybe every third time I load the mag.

I just checked one in my go bag that was doing it and it appears to be coming from the very bottom, supporting your theory!
Totally makes sense and resolves a riddle that I've been confounded by for almost 15 years!
