They finally feel sexy when they wear women's clothes even though they are just an unkempt, overweight, pale man.  At least that is what I am seeing in the Pacific NW.  Then they get so much positive feedback that they finally feel like they aren't a fat loser anymore.  So they cling to this little bit of positivity in their life.  

At least it gets them to shave their face and brush their hair, I suppose.
