@46cd5609 @ee534b4c Sorry if I’m stating the obvious but is your doctor updating your med-list in real time with you? As a locum I find myself doing this a fair bit, especially legacy medications not discontinued.

Another regular system error my wife (pharmacist) and I see is not updating the dose. 

Pharmacy faxes: “did you mean to go back to old dose”
Doc: scribbles “no” and faxes back

FF several months: Rx renewed again at old dose because chart not updated.
