It seems to fail when the zapping wallet doesn't wait more than a couple of seconds for the server to reply with an invoice. I don't run a node on AWS like all the pro-centralization folks on centralized LN hosted wallets. I see every attempted zap in logs, just the zapper quits before paying the invoice.
