My advice would be to look at the global posts and follow who you find interesting and the hash tags that you find interesting. This will give you a unique feed of posts. As you probably know there are no algorithms in nostr so it is more of a balance of when you check nostr and frequency of posts.

Follow heaps of accounts then you don't need to see the spam in global because your feed will be enough. If you set your feed to see conversations, you can discover people the people you follow are talking to. This creates a grass roots web of trust that is resistant to algorithmic manipulation.
