3rd world is always going to be challenging with wheelchair access but Id really recommend going to the Volcano lake near Santa Ana. It was the hallmark of my trip. Its like 2 hours NW of the Airport. You can stay there and find lots to do for several days. Look on travel sites and see the pictures. Wake up with a volcano outside your window and a lake. —Book early to get the rooms with views.— Amazing. Other highlights were El Tunco - beach The cathedral in San Salvador.
