It said if you are 18+ and want to enable nsfw content. How i understand this is that some might have nsfw content on there and you can enable the viewing of it on your end (so turned off by default). 
Seems more upfront than a lot of relays just having that stuff available by default. I’m not that familiar with the onlyfans business model, but isnt it people pay for nsfw content. This seems different
