Users can disable the reporting system at any time. Just hit the Security filters options in the menu and disable them. But the filter will always stay on by default. No one likes to see dick picks as soon as they log into a new app. 

Also, keep in mind that there are a lot of kids using Nostr already and adult content must be labeled as such so that clients can parse them out of a kid's feed.
